Medical Release Forms

**The Medical Release Form for Crossings 2008 will be available online no later than February 14th, 2008.**

Frequently Asked Questions about the Medical Release Form:

Why is the Medical Release Form online?

  • The Medical Release Form is now an online form that has been reformatted to better meet your needs.  The Medical Release Form is now being completed online because our insurance company is now requiring us to keep records until a student’s Statute of Limitations expires.  The Statute of Limitations expires on a camper’s 19th birthday.    Let’s take a typical 3rd grade who comes to Crossings for Kids, he/she is approximately 8-9 years old.  That means that our insurance is requiring us to keep the typical 3rd grader’s Medical Release Form approximately 10 years.  You can imagine that this would quickly become a mountain of paperwork, were it not digital.

Is the site secure?

  • The Medical Release Form is completed on a secure site.  This is the same encryption technology that many banks are using online.

I lost my form before giving it to my group leader. What do I do?

  • Should you lose this form or if you are not able to print out the form, your group leader will have access to print your medical release form.  Please contact them to acquire your new form

BLUEPRINT: A Radical Call to Faith and Obedience
Accepting the Gospel as truth is a life-changing decision.  Throughout the Sermon on the Mount, Jesus offers the truth and finally gives the challenge - build your house wisely on the rock of the Truth or build foolishly on the sand and do things your own way.

At Crossings 2008, we’ll meet the One who is Truth – the Architect, examine the plans He’s given, gather the supplies we need, and begin building on the Rock.

Matthew 7:24-27 (NIV)

"Therefore everyone who hears these words of mine and puts them into practice is like a wise man who built his house on the rock.
The rain came down, the streams rose, and the winds blew and beat against that house; yet it did not fall, because it had its foundation on the rock.
But everyone who hears these words of mine and does not put them into practice is like a foolish man who built his house on sand.

The rain came down, the streams rose, and the winds blew and beat against that house, and it fell with a great crash."
